1.Registration should be completed by applicant only . |
1.1.Legal name should be used (no nicknames) . |
2.Complete on-line application in its entirety. Incomplete applications will not be processed. |
3.When prompted select "Coach/Admin Registration". |
4.When prompted select "competitive" for play level. |
5.All approved passes will be vaild for 2 years (expiring All approved passes will be vaild for 2 years (expiring 8/15/2016) . |
6.$10 fee (payment by credit card only) . |
7.Do not send a photo. Please upload a headshot photo that will be printed on your pass. |
7.1.No hats or sunglasses should be worn. |

We have several changes this year which I believe will better improve our current Risk Management program. Everyone applying for risk management approval this year must continue to submit their social security number for a national background check. In addition, new applicants, as well as coaches with passes expiring 2019, must undergo a concussion awareness training and a sexual abuse awareness training. Both are done online at the applicant’s convenience.
The concussion awareness training takes about 20 minutes to go through while the sexual abuse awareness is approximately 60 minutes long. Both have an online examination which the applicant must take and pass. Upon completion of said training, certificates will be emailed to the user, which must then be uploaded to his/her Affinity account.
Concussion training must be done yearly while the Sex Abuse Awareness training is required every two (2) years.
Coaches or administrators who have both certificates will then be added to team rosters.
